BRI Care Consultation™ is a proven care-coaching solution delivered by telephone and email to adults with health conditions and their family or friend caregivers, empowering both to more effectively manage short- and long-term needs.  

BRI Care Consultation uses a combination of ongoing assessment, action planning and follow-up support to create real and lasting changes in the caregiving situation, resulting in a decrease in unmet needs, stress and strain, and symptoms of depression for both caregivers and the person receiving care.  

BRI Care Consultation helps to change the cycle of ongoing care, uncovering alternatives to current care methods, identifying helpful new community resources, mobilizing family and friend assistance, and driving lasting changes in the care situation.

Convenient, telephone-based sessions with a trained and experienced Care Consultant allow both care partners to address unmet needs, difficult care tasks and other challenges resulting from caregiving responsibilities. Realistic and achievable action steps are developed and adjusted in order to modify and improve the situation, thereby reducing negative impacts and enhancing positive aspects of caregiving.

The Care Consultant Information System (CCIS) is a secure, innovative web-based tool that guides trained Care Consultants throughout the delivery of the program. It allows Care Consultants to individually assess and record concerns for both caregivers and the person receiving care, easily and effectively monitor and update the action plan, and follow a schedule of ongoing contacts with participants.

Three-part framework for success

The model follows an established, proven protocol, but allows for flexibility to address the unique issues facing each individual caregiver and person receiving care.  

  1. Assessment: Care Consultants use Assessment to ensure that concerns are identified, addressed and prioritized using a gradual process that involves open dialogue between Care Consultants and individuals or families. 
  2. Action Plan: Care Consultants provide recommendations that help individuals and families move towards solutions to address identified problems, providing guidance on how to connect to appropriate resources, help with managing challenging behaviors or activities of daily living, communicating needs to health professionals, scheduling appointments and mobilizing support from other family and friends. 
  3. Ongoing Support: Through ongoing contact, Care Consultants provide outreach to individuals and families, follow-up on action step progress, identify new concerns and adjust activities in the Action Plan.

Two delivery models for flexible implementation

BRI Care Consultation Aligns with CMS GUIDE Model Requirements

BRI Care Consultation is one of six caregiver support programs endorsed by CMS for implementation by Participants in the GUIDE Model. This versatile and comprehensive program meets the caregiver support requirements outlined in CMS  guidelines.  

As a comprehensive program, BRI Care Consultation delivers person-centered care and meets the caregiver wherever they are in their care journey. Skilled Care Consultants (Care Navigators) help find tailored solutions for virtually any issue experienced by the caregiver or care receiver, including: 

  • Engaging family and friends in helping to provide care
  • Providing health- and care-related education 
  • Identifying and encouraging the use of proven community resources
  • Providing ongoing coaching, problem-solving and emotional support   

Our Care Consultation Information System houses your client records and captures all the data you’ll need for reporting. Built-in standard reports let you assess program metrics and quality at any time.

BRI Care Consultation Meets all CMS GUIDE Model Care Program Requirements

Comprehensive Assessment Separate for beneficiary and caregiver to identify needs The program addresses the unmet needs of both the beneficiary and their family or friend primary caregiver through a 35-point assessment of physical, social, and emotional needs; a 24-point assessment for the beneficiary and 11-point for the caregiver. 
Care Plan Beneficiaries receive care plan The care plan is a detailed Action Plan that creates a step-by-step roadmap of activities that move toward solutions to issues and concerns that are important to the beneficiary and caregiver. The Action Plan is regularly updated to reflect changes in needs and situations and sent to the beneficiary, caregiver, family members and friends who are involved in care. 
24/7 Access Member of care team or third-party representative Depending on staffing and capabilities of healthcare providers and/or healthcare systems implementing the GUIDE Model, the Care Consultation Information System and Consumer Portal can be used for 24/7 communication.

Caregiver Support

Caregiver education and support

Care Consultants provide quality education and information to caregivers on caregiving skills and best practices, strategies for managing cognitive and behavioral symptoms, available healthcare and community services, and emotional support. Moreover, the program empowers caregivers by teaching them how to solve problems as they emerge. The searchable Resource Library puts over 700 consumer-ready, vetted articles and videos at their fingertips to provide insight into virtually any topic.

Respite Services Care Consultants assist caregivers and beneficiaries with finding, arranging, and coordinating the use of Respite Services that meet their specific needs from a third-party provider-organizations.
Medication Management •    Clinician reviews and reconciles medication
•    Care navigator provides tips to maintain schedule
Care Consultants help with medication management by coaching caregivers on effectively communicating with medical providers and pharmacists, providing information on tips for using and monitoring medications, and accessing and using technology solutions for dispensing medications.  
Care Coordination and Transition Timely referral to specialists to address other health issues coordinated by Care Navigator Care Consultants assist with coordinating healthcare and community services, as well as support and assistance from family members and friends. They also provide coaching and direct assistance to the caregiver and beneficiary in finding, accessing, and monitoring specialty healthcare for co-existing medical conditions. Additionally, assistance is provided with decision making and arrangements for care transitions and changes in where the beneficiary resides.

Dr. David Bass is Senior Vice President, Senior Research Scientist and Director of the Center for Research and Education at Benjamin Rose Institute on Aging. He has led over 80 funded research projects on topics such as: coping with chronic illness; family caregiving; quality of care and services; dementia and dementia care; and use and impact of formal services and informal support. Results of these investigations have been widely published in scientific and clinical journals, such as the Journal of the American Geriatric Society and The Gerontologist. Results also have been used to develop innovative programs for older adults and their family members, such as BRI Care Consultation™, which currently is being delivered by over 50 licensed service organizations across the US.

His current research focuses on development, testing and implementation of evidence-based programs, with a special emphasis on care for persons with Alzheimer's disease or other dementias and their family caregivers, and caregiving among diverse populations. In accord with the mission of Benjamin Rose Institute on Aging, a consistent theme of Dr. Bass’ work is the translation of research into practical applications that enhance the wellness of, and quality of care for, older adults and their family members. Dr. Bass received his doctoral degree in Sociology from the University of Akron and Kent State University in 1985.

Branka Primetica is the BRI Care Consultation Program Manager for the Center for Research and Education at Benjamin Rose Institute on Aging. She has been part of the research team that developed BRI Care Consultation™ since 2001. Her responsibilities include: program marketing, licensing, serving as Master Trainer, providing ongoing technical support for BRI Care Consultation licensed sites, and managing day-to-day operations. Her background includes conducting research and working with family caregivers and at-risk older adults, including those with dementia and behavioral health problems. Primetica has played an integral role on several federal, state and locally funded initiatives that aimed to improve services for older adults and their family caregivers. She possesses experience in clinical practice as well as project management, including training, evaluation design, field operations, quantitative and qualitative data analysis, grant-writing and reporting. Primetica also serves as the Quality Improvement Director/Client Rights Officer for the Eldercare Services Institute, LLC at Benjamin Rose.

She earned her Bachelor’s Degree in Sociology with a Minor in Business Administration from Baldwin Wallace University and Master of Social Work from the Ohio State University.
